On the "Become a Vendor" page of this website, when you scroll down and see the document you should see this icon in the top right corner of the document. Click on that.

After that, a Google Doc should open where you can easily view the entire document. There should be a "File" button in the top left corner of this page. Click on that.

After clicking on the File button a dropdown will appear with multiple options. Look for the "Download" option; click or hover over that option.

After clicking or hovering over the download option, more options should appear. We are looking for the "PDF Document (.pdf)" option. Click on that and it should instantly download varying by browser. Some browsers will clearly show you where the download went, if you're having issues finding the download please move on to Step 5.

On Windows, head over to your File Explorer (Folder Icon in the Start Menu or Taskbar) and navigate to "Downloads" it should be the most recent download. You can see an example of a Windows computer below with the downloads folder open showing the application downloaded. From here you should be able to open and print it. 

If you have an application like Adobe Acrobat you should be able to fill it out with that application. If you lack an application like that just print the application off and fill it out by pen.
